Living At Home

Through a partnership with Fifty Forward Catholic Charities of Tennessee, Inc. offers assistance for elderly low income Davidson County residents. Case Management services are offered to help seniors maintain independent living by linking them with appropriate resources. Living at Home is a cooperative effort of several agencies. Case Manager assesses the elders’ needs and living conditions.  Appropriate referrals are made to maintain independent living and prevent premature nursing home placements. We help identify unmet needs and work with the person to have these needs met within their residence. There is no fee for the elder using the Living at Home service.
