Support Circles Program Forms

Monthly Update ~ To be completed in the first week of each month by Circle Leader and Mentor.

~ [Community Partner Agency volunteers] Because Support Circles is dependent on grants, sometimes we must provide photos and/or videos to show the progress of this program. We will not publish photos or videos of you without this release.

Informed Consent for Research ~ Support Circles is partnering with the Community Development and Research program out of Vanderbilt's Peabody College. This form contains details on that research component.

Friendship at the Margins, Chapter 1 ~ The first chapter of the book by Christopher Heuertz and Christine Pohl about discovering mutuality in service and mission. Together they show how friendship is a vocation that can bring reconciliation and healing to our broken world. When we befriend those on the margins of society by practicing hospitality and welcome, they say, we create communities where righteousness and justice can be lived out.
